One-Page Handout for Health Practitioners to Give Patients

A one-page GP handout is a simple printable or emailable resource that health professionals can give to patients and families when aged care decisions are becoming urgent.
 

It is designed for situations where:

• an older person is no longer safe at home

• a carer is overwhelmed

• there has been a fall, hospital admission or rushed discharge

• dementia, confusion or capacity concerns are emerging

• the family needs to contact My Aged Care

• aged care decisions are becoming urgent
   

The handout gives families a calm next step after the consultation. It explains that they are not expected to work everything out alone, directs them to My Aged Care and includes the link to the Who Cares? book as a practical guide to understanding aged care decisions in Australia.
